When you become a Participant with Keystone First Community HealthChoices (CHC), you will get an ID card. This is the card you will use to get the health care benefits and services that are covered by Medical Assistance. 

Which ID cards you get depend on what kind of health care coverage you have. The following are the different cards you may have, what they mean and when to use them.

When you have Medical Assistance and you have picked Keystone First CHC as your health plan...

There are 2 important cards you will get:

  1. Keystone First CHC ID card.
  2. ACCESS or EBT card.

You should have received your Keystone First CHC ID card in the mail. If you have not received your ID card yet, or if the card is lost or stolen, please call Participant Services at 1-855-332-0729 (TTY 1-855-235-4976). You can still get health care services while you wait for your card.

Your Keystone First CHC ID card is what you use to get the benefits and services you need that are covered by Medical Assistance.

Keystone First CHC ID cards issued before June 11, 2021, look like this:

You also will have an ACCESS or EBT card from the Pennsylvania Department of Human Services (DHS).

If you have not received your ACCESS or EBT card, or if it is lost or stolen, please call your case worker at your County Assistance Office for help. You can still get health care services while you wait for your card.

It’s important to always carry both cards with you. There are times when you will need to show both ID cards to get the Medical Assistance benefits and services you need.

When you have Medicare and Medical Assistance, and you have picked Keystone First VIP Choice as your Medicare health plan and Keystone First CHC as your Medical Assistance health plan…

There are 3 important cards you will get:

  1. Keystone First ID card (for both your Keystone First VIP Choice plan and your Keystone First CHC plan).
  2. Medicare card.
  3. ACCESS or EBT card.

You should have received your Keystone First ID card in the mail. If you have not received your ID card yet, or if it is lost or stolen, please call Participant Services at 1-800-450-1166 (TTY 711). You can still get health care services while you wait for your card. Your Keystone First ID card is what you use to get the benefits and services you need that are covered by either Medicare or Medical Assistance.

You should also have a Medicare card from the Centers for Medicare & Medicaid Services (CMS). This is sometimes called your “red, white, and blue” card.

Please do not carry your Medicare card with you. Keep it in a safe place. You will not need this card to get benefits and services, but you do need to keep it.

Finally, you should have an ACCESS or EBT card from the Pennsylvania Department of Human Services (DHS). If you have not received your ACCESS or EBT card, or if it is lost or stolen, please call your case worker at your County Assistance Office for help. You can still get health care services while you wait for your card.

It’s important to always carry all of your health plan ID cards and ACCESS or EBT card with you. There are times when you will need to show all ID cards to get the benefits and services you need.

Your BCBS ID Card Contains Vital Health Insurance Information

You can use the first three numbers on your member ID card to log into your account and access your benefits. 

Some items go everywhere with us. Our phones, keys, driver’s licenses and credit cards are kept at our fingertips at all times. When it comes to healthcare services, that item is your Blue Cross Blue Shield (BCBS) ID card.

Keep it in a safe, easily accessible place like your wallet. Your card contains key information about your health insurance coverage that is required when you need care or pick up a prescription at the pharmacy. Keeping your card in a secure location will also help protect you from medical identity theft.

If you have misplaced or haven’t yet received your BCBS ID card, a temporary ID card may be available online. Visit your local BCBS company’s website for details.

When your BCBS ID card arrives in the mail, take a moment to look for these five things:

1. Double-Check Details

Check your new card to ensure details like the spelling of your name and the type of plan you selected are correct. (See item #2 below to learn how to contact your BCBS company to address any errors.)

2. Key Contact Information

Your BCBS ID card has important contact information to help you reach your BCBS company. Look on the back of your card to find the phone numbers for customer service and eligibility questions. 

3. Your Member ID Number

Your BCBS ID card has your member number, and in some cases, your employer group number. Your member number, also known as your identification number, is listed directly below your name. You’ll need this information when receiving medical services at the doctor or pharmacy, or when calling customer service for assistance. If your group number is available, you'll find it directly below your member number. 

4. Your Plan Type

If you forget or aren’t sure what type of health insurance plan you have (like an HMO or PPO), you can find out on your BCBS ID card. If you have an HMO, your card may also list the physician or group you’ve selected for primary care. Determining whether a provider is in-network is an important part of choosing a primary care physician. 

5. Your Co-Pays

Your BCBS ID card may include a list of the co-pay amounts you can expect to pay for common services, like a doctor’s visit or trip to the emergency room.

The Blue Cross Blue Shield Association is an association of 35 independent, locally operated Blue Cross and/or Blue Shield companies.

What is group ID on insurance card?

Is group number the same as policy number?

Is policy number the same as group number? No, policy number is specifically for your policy, but group number usually refers to your employer or group where you're getting your coverage. If you have no group number on your insurance card, that usually just means you aren't receiving it through your employer.
